What it is, How it works

When gathered near the scalp, hair can offer an approximate 3-month detection period for detecting substances. Collecting hair is straightforward, relatively tamper-proof, and convenient to transport.

A sampled segment of 1.5 inches consisting of around 200 strands (similar to a #2 pencil in thickness) from the closest scalp area will yield 100mg of hair, which is optimal for screening and confirmation. For EtG, additional tests, and/or those exceeding 10 panels, a specimen of 150mg is advised. We suggest verifying the specimen weight using a jeweler’s scale. In case scalp hair isn't available, an equivalent amount of body hair may suffice. When referring to head hair, we imply scalp hair solely. Body hair encompasses all other hair variants (beard, underarm, etc.).

Overview of the Process

The four primary procedures in laboratory processing of drug testing results are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the primary handling of a specimen within a laboratory's framework. This comprises verification of proper sample sealing and shipping, allocation of a random LAN (Laboratory Accessioning Number), and filling any supplementary data not captured by an electronic chain of custody system.

Screening constitutes a preliminary quick assessment for substances of abuse. While Screening is a cost-efficient method to discount drug use in the majority of samples, a positive screen needs further confirmation for court admissibility. Any samples indicating presumptive positive results in Screening necessitate subsequent confirmation.

For samples presumed positive during Screening, additional hair is extracted from the initial specimen and readied for Extraction. In this segment, drugs are isolated from hair at notably lesser concentrations compared to other techniques (e.g., urine or oral fluid), signifying why hair drug screening is among the toughest methodologies to undertake.

Positive screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples indicating presumptive positive results are cleansed as needed. The full laboratory sequence, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation adhering to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
