What it is, How it works

Hair offers a prolonged historical record of alcohol and drugs by capturing biomarkers within the growing hair strand's fibers. Collected near the scalp, hair can show up to a 3-month detection range for alcohol and drugs. Hair collection is straightforward, relatively challenging to tamper with, and simple to transport.

A sample of 1.5 inches with approximately 200 hair strands (about the size of a #2 pencil) from the scalp yields 100mg, the preferred sample amount for screening and confirmation purposes. For EtG, supplementary analyses, and/or tests exceeding 10 panels, the recommended specimen amount is 150mg. It’s advisable to weigh the specimen using a jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair can be gathered. "Head hair" refers to scalp hair only, while "body hair" includes all other types (such as facial, axillary, etc.).

Process Overview

The drug test result processing in a laboratory follows four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of a sample’s entry into a laboratory's system. This process involves confirming the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entry, if not already provided by an electronic chain of custody system.

Screening refers to the initial rapid evaluation for drugs of abuse. Though Screening is an economical method to rule out drug use for most samples, a confirmed positive result is necessary for court admissibility. Any samples with a presumptive positive in Screening necessitate further confirmation.

In cases of presumptive positive results during Screening, additional hair is extracted from the initial specimen and set for Extraction. During Extraction, drugs are extracted from hair at considerably lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening more complex.

Confirmation of any positive results from Screening is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if needed. The entire lab process from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair designation and under ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
