What it is, How it works

Hair analysis has emerged as a significant method for identifying drug and alcohol consumption. Hair offers an extended history of substance use by encasing biomarkers within the hair fibers. When cut near the scalp, hair can indicate up to an estimated 3-month period for detecting alcohol and other substances. Hair is easy to gather, relatively challenging to tamper with, and straightforward to transport.

A 1.5-inch segment consisting of roughly 200 hair strands (comparable in size to a #2 pencil) adjacent to the scalp yields 100mg of hair, the optimal sample for testing and verification. For EtG, additional layers, and/or tests exceeding 10 panels, a 150mg sample is suggested. It is advisable to measure the specimen with a jeweler’s scale. If scalp hair cannot be obtained, a similar quantity of body hair may be collected. For head hair references, we mean scalp hair solely. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Summary

The procedure in a laboratory for a drug test outcome comprises four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the primary handling of a sample within the lab's framework. This step includes ensuring the sample was appropriately sealed and transported, allocating a unique LAN (Laboratory Accessioning Number), and finalizing any missing data entry not covered by an electronic custody system.

Screening consists of an initial rapid inspection for substance abuse. Although Screening is a cost-efficient approach to dismiss drug usage in many samples, positive screenings must undergo confirmation to be court-admissible. Samples with presumptive positives in Screening need further verification.

For a presumptive positive outcome in the Screening phase, additional hair is extracted from the original sample for Preparation for Extraction. Here, drugs are isolated from hair at much lower levels than other methods (such as urine or oral fluid), making hair drug tests the most complex to conduct.

Verification of any positive screenings occurs through GC/MS, GC/MS/MS, or LC/MS/MS. All preliminary positives are cleansed before verification as required. The entire lab process from Accessioning to Confirmation is examined in light of both the CAP (College of American Pathologists) Hair category and ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
