What it is, How it works

Hair analysis has gained recognition as a potent method for identifying drug and alcohol consumption. Hair captures a prolonged record of alcohol and other substances by encapsulating biomarkers within the strands of the hair as it grows. When taken near the scalp, hair can offer up to a roughly 3-month span for detecting alcohol and other substances. Hair is straightforward to collect, somewhat hard to tamper with, and simple to ship.

A 1.5-inch piece of around 200 strands of hair (comparable to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the perfect quantity for screening and verification. For EtG, add-ons, and/or tests surpassing 10 panels, 150mg of the sample is suggested. Measuring the sample with a jeweler’s scale is advisable. If scalp hair is lacking, a comparable portion of body hair may be utilized. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair forms (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome includes four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the early handling of a sample into a lab’s framework. This process includes ensuring the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry not encompassed by an electronic chain of custody system.

Screening serves as a preliminary rapid check for abusive drugs. While Screening is an economical way to eliminate drug use in most samples, a positive screen demands confirmation to be valid in legal proceedings. Any samples deemed presumptive positive during Screening require subsequent verification.

When a sample is likely positive in the Screening phase, more hair is extracted from the original sample and readied for Extraction. During Extraction, drugs are isolated from hair at a much reduced concentration than other techniques (ex. urine or oral fluid), which makes hair drug screening the most intricate methodology to execute.

Verification of any positive screening outcome is done via GC/MS, GC/MS/MS or LC/MS/MS. All samples suspected as positive are cleansed prior to verification if necessary. The entire lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
