What it is, How it works

Hair collection is straightforward, somewhat resistant to tampering, and easy to ship.

A sample measuring 1.5 inches, consisting of approximately 200 hair strands (similar to the diameter of a #2 pencil), closes to the scalp, yields 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG, add-ons, and tests exceeding 10 panels, 150mg of the specimen is advised. It is recommended to weigh the sample using a jeweler's scale. In the absence of scalp hair, an equivalent quantity of body hair can be collected. When mentioning head hair, we refer exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

There are four primary stages in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample into a lab's system. This step ensures the sample's seal and shipping are intact, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry absent from an electronic chain of custody system.

Screening consists of a quick initial check for substances of abuse. Although Screening is a cost-efficient way to rule out drug use in most samples, a positive result requires confirmation for court admissibility. Any samples that are presumptively positive in Screening necessitate a secondary confirmation.

If a sample tests presumptively positive in the Screening phase, additional hair is taken from the original specimen for Extraction. Here, drugs are extracted from hair at concentrations considerably lower than other methods (such as urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Positive screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptively positive are washed before confirmation as necessary. The full laboratory procedure from Accessioning to Confirmation undergoes review under the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
