A 1.5-inch sample of about 200 hair strands (comparable to the girth of a #2 pencil) nearest to the scalp provides 100mg of hair, considered the optimal sample for examination and validation. For EtG, supplemental tests, and/or tests exceeding 10 panels, 150mg of the sample is suggested. We advise measuring the specimen on a jeweler’s scale. Should scalp hair be unavailable, a comparable quantity of body hair can be gathered. "Head hair" specifically implies scalp hair. "Body hair" encompasses other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial processing of a sample within a laboratory’s system. This step verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and fulfills any additional data entry absence not addressed by an electronic chain of custody system.

Screening conducts an initial, swift examination for drugs of abuse. Though Screening is a budget-friendly method to exclude drug usage in most samples, a positive screen demands confirmation for court acceptability. Samples with presumptive positive results in Screening necessitate a secondary confirmation.

Should a sample be presumptively positive during the Screening phase, more hair is extracted from the initial specimen and readied for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than through other methods (e.g., urine or oral fluid), rendering hair drug testing the most challenging methodology to execute.

Confirmation of any positive Screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are subjected to washing before confirmation, as needed. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
