A sample of 1.5 inches consisting of about 200 hair strands (roughly the width of a #2 pencil) nearest to the scalp provides 100mg of hair, which is optimal for screening and verification. For EtG, additional testing, and/or panels exceeding 10, 150mg of the sample is advisable. It's recommended to use a jeweler’s scale to measure the specimen. If scalp hair is not accessible, a comparable amount of body hair can be collected. When addressing head hair, it specifically refers to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The core steps in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary preparation of a sample within the lab's framework. This step involves confirming proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. Although Screening is an economical approach to exclude drug usage on most samples, a positive screen requires confirmation for legal admissibility. Samples that test presumptively positive in Screening necessitate a follow-up confirmation.

For a presumptive positive result at the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are removed from hair at much lower concentrations than in other techniques (e.g., urine or oral fluid), establishing hair drug screening as the most challenging technique to execute.

Confirmation for any positive screening result is achieved through GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positives undergo washing prior to verification as needed. The entire lab procedure from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and holds accreditation to the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
