For EtG, add-ons, or tests exceeding 10 panels, 150mg of the hair specimen is advised. We suggest weighing the sample using a jeweler's scale. If scalp hair is not accessible, an equivalent quantity of body hair can be collected. When discussing head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four fundamental steps in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial processing of a sample into a lab's system. This step includes verifying that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening provides an initial rapid check for drug use. Although Screening is a cost-efficient method to exclude drug usage in most samples, a positive result requires confirmation to be admissible in court. Samples that screen presumptively positive require a confirmatory analysis.

If Screening indicates a presumptive positive, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than other methods (e.g., urine or oral fluid), affirming why hair drug screening is highly complex to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as necessary. The full laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
