What it is, How it works

When sampled close to the scalp, hair can potentially provide a detection period of around three months for alcohol and other substances. Hair is easy to gather, challenging to tamper with, and convenient to transport.

A hair sample measuring 1.5 inches and consisting of approximately 200 strands (roughly the diameter of a #2 pencil) taken nearest to the scalp ensures 100mg of hair—the optimal sample size for testing and verification. For EtG, enhancements, and/or tests beyond 10 panels, a 150mg sample is advisable. Using a jeweler’s scale for weighing the sample is suggested. If scalp hair is not present, a comparable amount of body hair can be used. The term head hair specifically refers to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The drug testing laboratory procedure encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the introduction of a sample into a lab’s system, ensuring the sample is appropriately sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry that an electronic chain of custody system may omit.

Screening involves a preliminary examination for drug abuse. Screening is an economical method to eliminate drug usage in most samples, but a positive screen must be verified for court admissibility. Any samples that test positive in Screening need secondary confirmation.

Should a sample test positive in Screening, additional hair is extracted from the main sample for the Extraction phase. In this phase, hair undergoes extraction of drugs at a concentration substantially lower than other techniques (e.g., urine or oral fluid), which is why hair drug testing is notably complex.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All positive samples undergo a washing process prior to confirmation as warranted. The complete laboratory process from Accessioning through to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
