What it is, How it works

Hair testing is widely recognized as an effective method for detecting drug and alcohol use. Hair offers a comprehensive timeline of alcohol and drugs by embedding biomarkers within the fibers of the growing strand. When taken near the scalp, hair can reveal drug and alcohol use over an approximate 3-month period. Hair collection is straightforward, fairly resistant to tampering, and simple to transport.

A sample consisting of a 1.5-inch length of roughly 200 hair strands (comparable to the size of a #2 pencil) nearest the scalp yields 100mg of hair, which is optimal for both screening and confirmation. For EtG, add-ons, or tests involving more than 10 panels, a 150mg specimen is advisable. We suggest using a jeweler’s scale for weighing the sample. If scalp hair isn't available, an equivalent amount of body hair should be used. By "head hair," we mean scalp hair alone. "Body hair" refers to every other type (facial, axillary, etc.).

Process Overview

The laboratory processing for a drug test result comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial sample integration into a laboratory’s system. This process checks whether the sample was appropriately sealed and transported, allocates a random LAN (Laboratory Accessioning Number), and completes any extra data entry omitted by an electronic chain of custody system.

Screening conducts an initial rapid assessment for drugs of abuse. Screening provides a cost-effective method to dismiss drug use in most samples; however, for legal admissibility, a positive result requires confirmation. Any samples with a presumptive positive in Screening necessitate a subsequent confirmation.

Should a sample be presumptive positive at the Screening stage, additional hair is extracted from the original specimen and prepared for Extraction. In Extraction, drugs are retrieved from hair at much lower concentrations than in other methods (like urine or oral fluid), underscoring the complexity of hair drug screening.

Confirmation of positive screening results is performed using GC/MS, GC/MS/MS or LC/MS/MS. Before confirmation, all presumptive positive samples undergo washing if needed. The overall lab process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
