A sample measuring 1.5 inches with roughly 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the perfect amount for both screening and confirmation purposes. For EtG, additional tests, and/or tests exceeding 10 panels, a sample size of 150mg is recommended. It’s advisable to weigh the sample using a jeweler’s scale. In cases where scalp hair is not available, an equivalent amount of body hair may be obtained. When we mention head hair, we are specifically referring to scalp hair only. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial incorporation of a sample into a lab’s system. This process includes confirming that the sample was properly sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and finishing any extra data entry not provided by an electronic chain of custody system.

Screening is a preliminary rapid assessment for potential drug use. Although Screening is a budget-friendly approach for excluding drug presence in the majority of samples, a positive screening outcome requires further confirmation to be valid in legal proceedings. Any samples with presumptive positive results in Screening necessitate an additional confirmation process.

If a presumptive positive is identified during Screening, additional hair is taken from the initial sample and prepared for Extraction. During this process, substances are retrieved from hair at a notably reduced concentration compared to other methods (e.g., urine or oral fluid), which contributes to hair drug screening’s reputation as the most complex method to execute.

Confirmation of any positive screening results is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo necessary wash procedures before confirmation. The whole laboratory procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
