For EtG, additional tests, and/or examinations beyond 10 panels, 150mg of the sample is suggested. We advise measuring the specimen on a jeweler’s scale. If scalp hair is unavailable, a comparable volume of body hair can be used. By head hair, we mean scalp hair exclusively. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory procedure for a drug test result consists of four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a sample into a lab’s system. This entails validating that the sample was sealed and shipped correctly, allocating a random LAN (Laboratory Accessioning Number), and completing any further data entry not supplied by an electronic chain of custody system.

Screening entails a preliminary rapid assessment for drugs of misuse. While Screening is a cost-efficient method for excluding drug use in most samples, a positive screen must be verified to be legally admissible. Any samples that are tentatively positive in Screening need a secondary confirmation.

If a sample is tentatively positive in the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. During this phase, drugs are extracted from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug analysis is the most challenging methodology to execute.

Confirmation of any positive screening result is conducted through GC/MS, GC/MS/MS or LC/MS/MS. All tentatively positive samples are cleaned before confirmation as required. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
