For EtG, additional evaluations, and/or tests beyond 10 panels, 150mg of the sample is advised. It is best to weigh the sample using a jeweler’s scale. If scalp hair is not available, a similar amount of body hair may be used. In references to head hair, we mean only scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The procedure for processing a drug test involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a specimen within the lab's workflow, including ensuring proper sealing and shipping of the sample, assigning it a LAN (Laboratory Accessioning Number) randomly, and fulfilling any additional data entry absent from an electronic chain of custody.

Screening is a preliminary assessment for substances of abuse. This cost-efficient measure helps exclude drug consumption in most specimens, but court-eligible results require confirmation. Samples that are potentially positive in Screening must undergo a second confirmation.

When a specimen tests potentially positive during Screening, more hair is extracted from the initial sample for the Extraction process. Here, substances are isolated from hair at much lower concentrations than other methods (e.g., urine or saliva), illustrating why hair testing is the most challenging technique to carry out.

GC/MS, GC/MS/MS, or LC/MS/MS methods conduct Confirmation of any positive Screening outcomes. Prior to Confirmation, all potentially positive samples are cleansed as necessary. The laboratory's entire procedure from Accessioning through Confirmation is overseen under the standards of both the CAP (College of American Pathologists) Hair classification and ISO/IEC 17025 accreditation.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
