What it is, How it works

Hair analysis is now recognized as an effective method for identifying drug and alcohol consumption. Hair retains a historical record of alcohol and drug use by embedding biomarkers in the fibers of the hair as it grows. A sample taken near the scalp can offer a detection window of up to approximately three months for alcohol and drugs. Hair collection is straightforward, relatively tamper-resistant, and simple to transport.

A sample measuring 1.5 inches and consisting of about 200 hair strands (around the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG tests, add-ons, and/or tests above 10 panels, 150mg is advised. We suggest using a jeweler's scale to weigh the sample. If scalp hair isn't accessible, a similar quantity of body hair may be gathered. When mentioning head hair, we are specifically referring to scalp hair. Body hair encompasses other hair types (such as facial or axillary hair).

Process Overview

The laboratory processing of a drug test is primarily composed of four stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory's procedures. This process includes confirming that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and finalizing any additional data entry not supplied by an electronic chain of custody system.

Screening involves a preliminary rapid check for the presence of drugs of abuse. While Screening is a cost-efficient approach to eliminate drug use for most samples, any positive screen requires confirmation to be valid in court. Samples that are preliminarily positive in Screening require further confirmation.

In cases where a sample tests presumptively positive at the Screening stage, additional hair is extracted from the initial sample and readied for Extraction. Here, drugs are extracted from hair at significantly lower concentrations compared to methods like urine or oral fluid tests, making hair drug testing a challenging technique.

Confirmation of any positive screening is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are preliminarily positive are washed prior to confirmation if needed. The complete laboratory process from Accessioning to Confirmation is inspected under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
