What it is, How it works

Hair serves as a record of alcohol and drug activity by encasing biomarkers within the hair's structure. When sampled near the scalp, hair can offer a detection timeframe of around three months for substances. Collecting hair is straightforward, relatively tamper-proof, and convenient to transport.

To achieve an optimal test sample, procure a 1.5-inch section comprising approximately 200 hairs (roughly the thickness of a #2 pencil) adjacent to the scalp, yielding 100mg of hair. For tests such as EtG, add-ons, or those exceeding 10 panels, a 150mg sample is advised. It's best to measure samples on a jeweler’s scale. Should scalp hair be unavailable, a similar quantity of body hair is acceptable. Note that "head hair" pertains solely to scalp hair, whereas "body hair" refers to all other types, including facial and axillary.

Process Overview

The laboratory processes for drug testing encompass four pivotal steps: Accessioning, Screening, Extraction, and Confirmation.

The Accessioning step involves initially recording the sample in a lab's system. This process includes ensuring the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and finalizing any data entry not covered by an electronic chain of custody system.

During Screening, an initial assessment is made for potential drug presence. While a cost-effective method to dismiss drug use for most samples, any positive screening results require subsequent confirmation to be legally recognized. Screening presumptive positives must be verified.

If Screening indicates a presumptive positive, additional hair is extracted from the original sample for further preparation in the Extraction stage. Here, drugs are extracted from hair at much subtler levels than in other approaches (e.g., urine or oral fluid), explaining why hair tests are notably challenging.

Confirmation of any initial positive result is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positives undergo washing before confirmation as necessary. The entire procedure, from Accessioning to Confirmation, adheres to the CAP (College of American Pathologists) Hair criteria and the ISO/IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
