A 1.5-inch lock of around 200 hair strands (a #2 pencil's width) near the scalp yields 100mg, which is the optimal sample size for screening and confirmatory testing. For tests involving EtG, add-ons, or over 10-panel screenings, 150mg of the hair specimen is advisable. It is suggested to weigh the sample using a jeweler's scale. In instances where scalp hair is unobtainable, a comparable volume of body hair may be used. References to head hair mean scalp hair exclusively. Body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The lab analysis for a drug test result comprises four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary phase where the sample is introduced into the lab's tracking system. This step includes ensuring the sample was properly sealed and transported, generating a random LAN (Laboratory Accessioning Number), and completing any required data input not covered by an electronic custody system.

Screening is a preliminary assessment for drugs. Although it's a cost-effective means for excluding drug traces in most specimens, positives discovered in Screening necessitate validation to be legally credible. Presumed positive samples in Screening require additional verification.

If Screening yields a presumed positive, additional hair is taken from the primary sample for Extraction. In Extraction, drugs are isolated from hair in significantly lower concentrations compared to other methods (such as urine or saliva), making hair drug screenings the most challenging to conduct.

Positive findings in Screening require Confirmation through techniques like GC/MS, GC/MS/MS, or LC/MS/MS. Any presumed positives are cleansed before confirmation as required. The entire lab process, from Accessioning to Confirmation, undergoes review under the CAP (College of American Pathologists) Hair certification and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
