Gathering a 1.5-inch sample containing approximately 200 hair strands (similar to the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the desired quantity for initial screening and confirmation tests. For EtG, additional tests, or screenings encompassing more than 10 panels, a sample weight of 150mg is advocated. We advise using a jeweler’s scale for measuring the specimen. In cases where scalp hair isn't obtainable, an equivalent quantity of body hair is usable. When we mention head hair, we are specifying scalp hair solely. Body hair encompasses all other forms like facial and axillary hair.

Process Overview

Four fundamental steps make up the lab process for obtaining drug test results: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary process of logging a sample into a laboratory’s system. This stage verifies that the sample was appropriately sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry duties not covered by a digital chain of custody system.

Screening serves as an initial swift assessment for drug substances. Although Screening is an economical approach to exclude drug consumption for most samples, a positive screening requires confirmation to be legally valid in court. Samples with presumptive positives from Screening must undergo further confirmation.

If Screening indicates a presumptive positive, additional portions of the hair specimen are prepared for Extraction. In this phase, drugs are derived from hair at significantly lower concentrations than other methods (such as urine or oral fluid), underscoring the complexity of performing hair drug screening.

To authenticate a positive screening result, methods like GC/MS, GC/MS/MS, or LC/MS/MS are employed. Presumptive positive samples are cleansed before confirmation as necessary. The entire laboratory process, from Accessioning through Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
