Typically, 1.5 to 2 inches of hair on the head or body can be used. Analysis includes drugs, alcohol/ETG, or other tailored hair testing solutions.

Recent years have seen an increase in conducting drug tests via hair follicle analysis. Employers, judicial systems, and Substance Abuse experts now often favor hair drug tests over standard urine tests. These follicle screenings are ideal for zero-tolerance policies, court purposes, and probation monitoring. Advantages include an extended detection window, normally up to 90 days, stringent collection procedures, and resistance to tampering. Nonetheless, for identifying recent drug use within 5 days, urine tests remain the most precise option.

What it is, How it works

Hair testing is increasingly recognized as a valuable method for identifying drug and alcohol use. The growing hair strand traps biomarkers, providing a long-term history of substance use. When collected near the scalp, hair can offer a detection window of up to 3 months for alcohol and other drugs. Hair is easy to collect, challenging to alter, and simple to transport.

A sample of 1.5 inches consisting of approximately 200 hair strands (around the width of a #2 pencil) from the scalp yields 100mg of hair, which is the ideal amount for screening and confirmation processes. For EtG, additional tests, and/or tests beyond 10 panels, it's recommended to collect 150mg of the specimen. We suggest weighing the sample using a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be used. When we mention head hair, it is specifically referencing scalp hair, while body hair includes all other types (such as facial and axillary hair).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is entered into a laboratory’s system. This step includes ensuring the sample was properly sealed and transported, generating a random LAN (Laboratory Accessioning Number), and completing any additional data entry if not provided by an electronic chain of custody system.

Screening is a preliminary check for drugs of abuse. While it is a cost-effective approach to rule out drug use in most samples, any positive result from Screening must be confirmed to be usable in court. All samples that test presumptively positive in Screening require a secondary confirmation process.

When a sample tests positive in the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. In this stage, drugs are extracted from the hair at a significantly lower concentration compared to other methods (such as urine or oral fluid). This makes hair drug screening the most challenging methodology to perform.

Confirmation of positive screening results is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair designation as well as accreditation to ISO/IEC 17025 standards.

Advantages of hair drug testing:

  • Long detection window: Hair drug tests can identify drug use for up to 90 days, unlike urine tests that have a shorter detection window.
  • Difficulty to cheat: Cheating a hair drug test is quite difficult, leading to more reliable results.
  • Provides historical data: It offers a pattern of drug use over a time frame, not just recent use.

Limitations:

  • Cannot detect recent use: Drugs become detectable in hair after approximately 5-7 days.
  • Cost: Hair drug tests are typically more costly compared to other drug testing methods.
  • Variations in results: Factors like hair color and individual hair growth differences can influence drug metabolite concentration in the hair.

Note: Despite being commonly referred to as "hair follicle tests," the test analyzes the hair strand, not the follicle under the scalp.
