For EtG, additional tests, or panels exceeding 10, a sample of 150mg is advised. It is recommended to weigh the sample using a jeweler’s scale. If scalp hair is not accessible, a corresponding quantity of body hair can be used. When mentioning head hair, we mean scalp hair specifically. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into the laboratory’s system. This step involves ensuring the sample was correctly sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and fulfilling any extra data entry not provided through an electronic chain of custody system.

Screening consists of a preliminary review for substances of abuse. Although Screening is a cost-saving method to dismiss drug use in most samples, a positive screen must be validated for court acceptance. Any samples presumed positive in Screening mandate subsequent confirmation.

Confirmation for any positive screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to Confirmation, all samples presumed positive are cleansed as necessary. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
