When obtained from near the scalp, it can reveal alcohol and drug use for around three months. Hair is easy to collect, challenging to tamper with, and straightforward to send.

A 1.5-inch segment comprised of approximately 200 hair strands (similar in size to a #2 pencil) taken from the scalp gives 100mg of hair, which is ideal for both screening and confirmation. For EtG, additional analyses, and/or evaluations beyond 10 panels, 150mg of the sample is advised. Consider weighing the sample with a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair can be gathered. When discussing head hair, we refer solely to scalp hair. Body hair includes all other types such as facial and axillary hair.

Process Overview

The procedure for analyzing a drug test outcome in the lab involves Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the laboratory system. This step entails verifying the sample was sealed and transported correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry not provided by an electronic chain of custody system.

Screening provides a preliminary check for drug presence. Although Screening is an economical approach to exclude drug usage for most samples, legal proceedings require confirmation of a positive screen. Any samples showing a tentative positive result during Screening need further confirmation.

Should a sample appear positive in Screening, more hair is extracted from the initial sample for the Extraction phase. This phase involves retrieving drugs from the hair at much lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug testing a complex procedure.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS or LC/MS/MS. Samples with presumptive positives are washed before confirmation as necessary. The laboratory process from Accessioning through Confirmation is subjected to review under the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 accreditation standards.
