What it is, How it works

When collected near the scalp, hair offers a detection window of up to roughly 3 months for alcohol and other substances. Hair is easy to collect, relatively hard to tamper with, and simple to transport.

A 1.5-inch segment of roughly 200 hair strands (comparable to a #2 pencil) closest to the scalp provides 100mg of hair, which is the optimal amount for analysis and verification. For EtG, additional tests, and/or screenings beyond 10 panels, a sample of 150mg is advisable. We suggest weighing the hair on a jeweler’s scale. If scalp hair is not present, a similar quantity of body hair can be used. When mentioning head hair, we mean only scalp hair. Body hair includes all other varieties (facial, axillary, etc.).

Process Overview

There are four key stages within the lab processing of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial handling of a sample into a lab’s system. This step includes verifying the sample was properly sealed and sent, assigning it a random LAN (Laboratory Accessioning Number), and completing any data entry not managed by an electronic chain of custody system.

Screening is an introductory quick scan for drugs of abuse. While Screening offers a cost-efficient method to eliminate drug use on most samples, a positive outcome must be confirmed for court admissibility. Any sample showing a presumptive positive in Screening requires a secondary verification.

If a specimen is presumptively positive during the Screening phase, additional hair is taken from the original sample and prepared for Extraction. At this stage, drugs are isolated from hair at a significantly lower concentration compared to other techniques (e.g., urine or oral fluid), making hair drug analysis one of the toughest methodologies to execute.

Verification of any positive screening result is performed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The complete lab process from Accessioning to Confirmation is reviewed under both CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
