Hair collection is straightforward, relatively resistant to tampering, and easy to dispatch.

A 1.5-inch segment consisting of roughly 200 hair strands (approximately the width of a #2 pencil) nearest the scalp yields 100mg, the optimal sample weight for assessment and verification. For EtG, add-ons, and/or examinations exceeding ten panels, a specimen weight of 150mg is advisable. We advise determining the specimen's weight using a jeweler’s scale. If scalp hair cannot be obtained, an equivalent quantity of body hair may be used. "Head hair" specifically denotes scalp hair. "Body hair" encompasses all other hair types (e.g., facial, axillary, etc.).

Process Overview

The quartet of primary procedures in processing a laboratory drug test result includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial intake of a sample into a laboratory’s system. This step includes confirming proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any supplementary data entry not supplied by an electronic chain of custody system.

Screening provides an initial rapid assessment for drug abuse. While Screening offers a cost-efficient method to exclude drug usage in most samples, a positive result mandates confirmation for court admissibility. Any specimens presumptively positive during Screening must undergo a secondary confirmation.

Should a specimen test presumptively positive in the Screening phase, additional hair is gathered from the original sample and readied for Extraction. Here, the extraction of drugs from hair occurs at concentrations significantly lower than those in other testing methodologies (like urine or oral fluid), marking hair drug screening as one of the most complex methodologies to execute.

Confirmation of any positive screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive specimens undergo washing before confirmation where necessary. The comprehensive laboratory sequence from Accessioning to Confirmation is scrutinized under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
