What it is, How it works

Hair analysis has gained recognition as an effective method for detecting drug and alcohol consumption. When obtained near the scalp, hair can reveal up to a 3-month detection period for alcohol and other substances. Hair is easy to collect, challenging to tamper with, and straightforward to transport.

A 1.5-inch segment of roughly 200 hair strands (comparable to the size of a #2 pencil) closest to the scalp will provide 100mg of hair, which is the optimal sample for screening and validation. For EtG, additional tests, and tests exceeding 10 panels, 150mg of the sample is recommended. It's advisable to weigh the sample on a jeweler’s scale. If scalp hair is not available, a similar amount of body hair may be taken. When referring to head hair, we mean scalp hair specifically. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The four primary steps in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial handling of a sample into a laboratory’s system. This process covers confirming the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any extra data entry that the electronic chain of custody system does not provide.

Screening involves an initial quick check for drugs of abuse. While Screening is a cost-effective means to exclude drug usage for most samples, a positive screen must be validated to be court-admissible. Samples that are presumptively positive in Screening necessitate a secondary confirmation.

Should a sample be presumptively positive at the Screening phase, more hair is extracted from the original sample and prepared for Extraction. In this phase, drugs are extracted from hair at a much lower concentration than through alternative methods (like urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening result is completed through GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as needed. The complete laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
