A snippet measuring 1.5 inches of approximately 200 hair strands (comparable to a #2 pencil in thickness) nearest to the scalp yields 100mg, which is the optimal sample for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a specimen of 150mg is advised. It's recommended to weigh the sample using a jeweler’s scale. If scalp hair is not accessible, a comparable amount of body hair may be taken. When referring to head hair, we mean scalp hair exclusively. Body hair refers to other types of hair (facial, axillary, etc.).

Process Overview

The four principal phases in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial processing into the laboratory's system. This involves confirming that the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry missing from an electronic chain of custody system.

Screening constitutes an initial rapid assessment for substances of abuse. While Screening is a cost-efficient method to dismiss drug usage in most samples, a positive screen must be confirmed to be court admissible. Any samples flagged as presumptively positive in Screening necessitate a secondary confirmation.

Should a sample appear presumptively positive in the Screening phase, additional hair is extracted from the original specimen and readied for Extraction. In this phase, substances are extracted from hair at a much lower concentration compared to other techniques (e.g., urine or oral fluid), making hair drug screening one of the most challenging methodologies.

Confirmation of any positive Screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The entirety of the laboratory process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
