Hair testing is recognized as an effective means for identifying alcohol and drug usage. Hair strands capture a historical timeline of drug and alcohol consumption by embedding biomarkers within the fibers. When hair is collected near the scalp, it can offer a detection range of roughly three months for alcohol and other substances. The collection of hair is straightforward, challenging to tamper with, and simple to transport.

Collecting a 1.5-inch section of about 200 strands of hair (comparable to the thickness of a #2 pencil) nearest to the scalp will yield around 100mg of hair, which is the perfect amount for both screening and confirmation. For EtG, additional screenings, or tests exceeding 10 panels, it is recommended to gather 150mg of the sample. We suggest weighing the sample using a jeweler’s scale. If hair from the scalp is not available, a similar amount of body hair can be gathered. "Head hair" denotes only scalp hair. "Body hair" includes all other hair categories (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary procedure whereby a sample is entered into the laboratory’s system. This step consists of ensuring the sample was sealed and sent properly, generating a random LAN (Laboratory Accessioning Number), and inputting any extra data not included in an electronic chain of custody system.

Screening is a preliminary examination for the presence of drugs. Although Screening is an economical method to eliminate drug use in most samples, a positive screen must be confirmed to hold up in legal settings. Any samples indicating a positive result in Screening mandate a subsequent confirmation.

If a specimen shows a positive indication during Screening, additional hair is extracted from the original sample for Extraction. During this phase, the drugs are extracted from the hair at a notably lower concentration compared to other methods (such as urine or saliva), which is why drug detection in hair is the most challenging methodology to execute.

Positive results from Screening are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples are washed beforehand as required. The entire laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 standards.
