A 1.5-inch sample containing around 200 strands of hair (about the circumference of a #2 pencil) taken nearest to the scalp will yield 100mg of hair, which is the preferred amount for testing and verification. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is suggested. It is advisable to measure the sample with a jeweler’s scale. If scalp hair cannot be obtained, a comparable quantity of body hair can be gathered. When mentioning head hair, we only mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The key stages in the laboratory handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial process of a sample into a lab's system. This includes ensuring the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening involves a preliminary rapid examination for drugs of abuse. While Screening is a cost-effective method to exclude drug usage in most samples, a positive screen must be confirmed for court admissibility. Any samples suggesting positive results in Screening need further confirmation.

If a sample indicates positive results in the Screening phase, additional hair is taken from the original sample for Extraction preparation. In this phase, drugs are extracted from hair at lower concentrations than in other methods (such as urine or saliva), making hair drug screening the most complex methodology to execute.

Confirmation of positive screening results is completed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples suggesting positive results are washed before confirmation as required. The entire process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
