What it is, How it works

When procured near the scalp, hair can reveal up to a roughly 3-month timeframe for detecting alcohol and drugs. Collecting hair is straightforward, moderately challenging to tamper with, and straightforward to transport.

A sample measuring 1.5 inches consisting of approximately 200 hair strands (akin to the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal amount for both screening and confirmation. For EtG, additional tests, and/or analyses surpassing 10 panels, a quantity of 150mg is advised. We suggest utilizing a jeweler’s scale to weigh the specimen. If scalp hair cannot be collected, a comparable quantity of body hair is acceptable. When referencing head hair, this pertains strictly to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The central stages in the laboratory analysis of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the introductory phase where a sample is integrated into a laboratory's framework. This step involves confirming that the sample was sealed and dispatched correctly, assigning an arbitrary LAN (Laboratory Accessioning Number), and finalizing any extra data entry absent from an electronic chain of custody system.

Screening constitutes an initial rapid evaluation for illicit drugs. Although Screening is an affordable method to rule out drug usage in most samples, a positive screen necessitates confirmation for court admissibility. Samples deemed presumptively positive in Screening do demand a subsequent confirmation.

Should a sample indicate a presumptively positive result during the Screening phase, additional hair from the initial sample is selected for Extraction. In this phase, drugs are extracted from hair at a noticeably lower concentration compared to other methods (such as urine or oral fluid), which underscores the complexity of performing hair drug screening.

Confirmation of any positive Screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicating a presumptive positive result are cleansed before confirmation as necessary. The entire lab process from Accessioning through to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
