A 1.5-inch segment consisting of around 200 hairs (comparable to the circumference of a #2 pencil) near the scalp will provide 100 mg of hair, the ideal amount for screening and confirmation. In the case of EtG, add-ons, and/or tests exceeding 10 panels, 150 mg of hair is advisable. We suggest weighing the hair on a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair may be used. When we mention head hair, we mean only scalp hair. Body hair refers to all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial integration of the sample into the laboratory’s system. This entails verifying the sample's seal and correct shipment, assigning a random Laboratory Accessioning Number (LAN), and completing any additional data entry not covered by an electronic chain of custody system.

Screening performs an initial rapid check for drugs of abuse. While Screening is a cost-effective method to exclude drug use in the bulk of samples, a positive screen must be confirmed for legal acceptance. Any samples that are presumptively positive during Screening necessitate further confirmation.

When a sample yields a presumptive positive in Screening, additional hair is extracted from the original sample and readied for Extraction. In this phase, drugs are isolated from hair at a considerably lower concentration than with other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a wash prior to confirmation as required. The complete laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
