A hair sample measuring 1.5 inches and containing about 200 strands (similar to a #2 pencil's thickness) cut close to the scalp gives 100mg of hair, which is ideal for both screening and confirmation tests. For EtG, additional tests, or those exceeding 10 panels, a 150mg sample is preferable. It's advisable to weigh the hair using a jeweler’s scale. If scalp hair is missing, an equivalent quantity of body hair may be used. References to head hair specifically mean scalp hair, while body hair encompasses other types such as facial and axillary.

Process Overview

The key stages in processing a drug test result in the lab include Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, a sample is initially processed into the lab’s system. This step ensures that the sample was sealed and transported correctly, a random LAN (Laboratory Accessioning Number) is assigned, and any further data entry not handled by an electronic chain of custody system is completed.

Screening serves as an early check for substance abuse. While it is a cost-effective strategy to eliminate drug presence in most cases, any positive outcomes require verification for legal admissibility. Samples that screen positive undergo further confirmation.

If a Screening indicates a positive result, more hair is extracted from the original sample for further Extraction. In this process, drugs are isolated from hair in much lower concentrations than with methods like urine or oral fluid tests, making hair drug screening notably challenging.

Confirmation for any positive screen result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, presumptive positive samples are cleansed as necessary. The entire lab operation, from Accessioning to Confirmation, is assessed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
