Hair collection is straightforward, fairly resistant to tampering, and simple to transport.

A 1.5-inch section of approximately 200 hair strands (comparable to the thickness of a #2 pencil) closest to the scalp yields 100mg, which is the optimum sample for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advisable. It is recommended to weigh the sample using a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair may be gathered. When indicating head hair, we mean scalp hair specifically. Body hair encompasses all other varieties (facial, axillary, etc.).

Process Overview

The four key stages involved in processing a drug test result in a lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the preliminary integration of a sample into a laboratory's system. This process verifies that the sample was adequately sealed and dispatched, assigns a unique LAN (Laboratory Accessioning Number), and completes any necessary data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid evaluation for substances of abuse. Although Screening is a cost-efficient method to rule out drug presence in the bulk of samples, a positive screen must be verified to be admissible in court. Presumptive positive samples from Screening do necessitate secondary confirmation.

If a sample shows presumptive positivity at the Screening stage, additional hair is retrieved from the original specimen and prepared for Extraction. At this phase, drugs are extracted from hair at a significantly reduced concentration compared to other methods (e.g., urine or oral fluid), which is why hair drug screening poses the greatest methodological challenge.

Confirmation of any positive Screening outcome is executed through GC/MS, GC/MS/MS or LC/MS/MS. All presumptively positive samples are cleansed before confirmation as appropriate. The complete laboratory process from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair designation and accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
