What it is, How it works

Hair analysis is recognized as an effective means for identifying drug and alcohol consumption. Hair provides an extended timeline of alcohol and other drug usage by embedding biomarkers within the fibers of the hair strand as it grows. When cut close to the scalp, hair can reveal a detection period of up to about 3 months for alcohol and drugs. Hair is easy to gather, somewhat tough to tamper with, and simple to transport.

A hair sample that is 1.5 inches long with approximately 200 strands (around the size of a #2 pencil) nearest to the scalp yields 100mg, the preferred sample for both screening and confirmation. For EtG, additional tests, and tests over 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale for weighing the sample. If head hair is unavailable, a comparable quantity of body hair can be used. 'Head hair' refers specifically to scalp hair, whereas 'body hair' encompasses all other types, such as facial and axillary hair.

Process Overview

The four essential phases in processing a drug test in the lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial processing of a specimen into a lab's system. This process encompasses confirming the sample's correct sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. Although Screening is cost-efficient for eliminating drug usage in most samples, a positive screen requires confirmation to be legally acceptable. Any samples preliminarily positive in Screening do require further confirmation.

If a sample is preliminarily positive during the Screening phase, more hair is extracted from the original sample and prepared for Extraction. In this stage, drugs are retrieved from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most complex method to execute.

The Confirmation of any positive screening result is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All preliminarily positive samples are cleaned prior to confirmation as necessary. The entire lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
