A sample measuring 1.5 inches, consisting of roughly 200 hair strands (similar in size to a #2 pencil) nearest the scalp, produces 100mg of hair, which is the ideal sample for analysis and verification. For tests involving EtG, add-ons, and/or more than 10 panels, 150mg of the sample is advised. It is recommended to measure the sample using a jeweler’s scale. If scalp hair is not available, a comparable amount of body hair can be used. Reference to head hair indicates scalp hair solely. Body hair encompasses all other types of hair (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample into a lab’s system. It includes confirming that the sample was sealed and transported properly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input not supplied by an electronic chain of custody system.

Screening involves an initial rapid check for drugs of abuse. Although Screening is a cost-efficient method to exclude drug use in most samples, a positive result requires confirmation for court admissibility. Any samples that test presumptively positive in the Screening phase require further confirmation.

If a sample tests positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this stage, drugs are removed from hair at much lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to perform.

Confirmation of positive screening results is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All samples identified as presumptively positive undergo washing before confirmation if necessary. The entire laboratory procedure, from Accessioning to Confirmation, is audited under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.
