What it is, How it works

Hair testing is recognized as an effective method for detecting drug and alcohol use. It provides a long-term historical record of these substances by embedding biomarkers within the hair's fibers as it grows. When taken near the scalp, hair offers an approximate 3-month detection timeframe for alcohol and other drugs. Hair collection is straightforward, relatively tamper-resistant, and simple to transport.

A 1.5-inch section consisting of about 200 hair strands (roughly the thickness of a #2 pencil) from the scalp can yield 100mg of hair, which is ideal for both screening and confirmation. For EtG, additional tests beyond 10 panels require 150mg of hair. We advise weighing the sample using a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair may be gathered. "Head hair" specifically refers to scalp hair, while "body hair" includes all other types (like facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial workflow of a sample in a laboratory system. This step involves ensuring the sample was properly sealed and transported, assigning a LAN (Laboratory Accessioning Number), and completing any missing data entry not supplied by an electronic chain of custody system.

Screening is an initial assessment for the presence of drugs. It serves as a cost-effective measure to negate drug use in most samples, although a positive screening result must be confirmed for legal validation. Samples with a presumptive positive screen require further confirmation.

If the Screening phase indicates a presumptive positive, additional hair from the original sample is prepared for Extraction. This involves extracting drugs from hair at much lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening a challenging procedure.

Confirmation of positive screenings is performed via GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumed positives are washed before confirmation if necessary. The complete laboratory process from Accessioning to Confirmation adheres to both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
