A 1.5-inch segment from around 200 hair strands (nearly the diameter of a #2 pencil) near the scalp affords 100mg, which is optimal for screening and corroboration. For EtG, supplements, and/or analyses exceeding 10 panels, 150mg of the sample is advised. It is suggested to measure the specimen using a jeweler’s scale. If scalp hair cannot be collected, a similar amount of body hair can be used. Reference to head hair implies just the scalp hair. Body hair encompasses all other hair types including facial and axillary hair.

Process Overview

The lab-based drug test result process includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial sample integration into a lab’s system. It verifies that the sample is correctly sealed and shipped, involves assigning a random LAN (Laboratory Accessioning Number), and requires completing any additional data entry not available via an electronic chain of custody system.

Screening constitutes a preliminary swift check for substance usage. While Screening is an economical way to eliminate drug use for most samples, a positive result needs verification for court acceptance. Presumptively positive samples during Screening demand further confirmation.

Should a sample yield a presumptive positive during Screening, extra hair from the initial collection is employed for Extraction. This stage entails removing drugs from hair at significantly lower concentrations compared to other methods (like urine or oral fluid), thus making hair drug analysis the most arduous to execute.

All positive screening results undergo confirmation employing GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, presumptively positive samples are washed if necessary. The entire lab procedure, spanning from Accessioning to Confirmation, is evaluated based on both the CAP (College of American Pathologists) Hair criteria and compliance with ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
