What it is, How it works

Hair captures a historical record of alcohol and drug intake by embedding biomarkers within the fibers of the developing hair strand. When taken near the scalp, hair can offer up to a roughly 3-month window for detecting alcohol and drugs. Hair is easy to gather, relatively hard to tamper with, and straightforward to transport.

A 1.5-inch segment of about 200 hair strands (roughly the diameter of a #2 pencil) closest to the scalp will provide 100mg of hair, the optimal sample size for screening and confirmation. For EtG add-ons and/or tests beyond 10 panels, 150mg of the sample is suggested. We advise weighing the sample on a jeweler's scale. If scalp hair is not available, a similar amount of body hair can be used. When mentioning head hair, we mean scalp hair exclusively. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary steps in a drug test outcome's laboratory processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the preliminary processing of a sample into a laboratory's operations. This entails confirming that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any remaining data entry missing from an electronic chain of custody system.

Screening involves an initial quick assessment for abuse substances. Screening is an economical way to exclude drug use in most samples; however, a positive result requires confirmation for admissibility in court. Any samples that test presumptively positive in Screening must undergo a secondary confirmation.

If a sample tests presumptively positive during the Screening phase, more hair is extracted from the original sample and readied for Extraction. At this stage, drugs are extracted from the hair at much lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug testing the most challenging method to perform.

Confirmation of any positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The complete laboratory procedure from Accessioning to Confirmation is inspected under the CAP (College of American Pathologists) Hair specification and the ISO / IEC 17025 accreditation standards.
