What it is, How it works

A 1.5-inch portion containing about 200 strands of hair, roughly equating to the size of a #2 pencil, nearest to the scalp, yields 100mg of hair, which is ideal for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale to weigh the sample. If no scalp hair is available, a similar amount of body hair can be collected. "Head hair" refers strictly to scalp hair. "Body hair" encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

Four essential steps in laboratory drug test processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial sample processing into a lab's system. It involves ensuring the sample was correctly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening is an initial quick drug abuse assessment. While Screening economically rule out drug use in most cases, a positive screen needs confirmation for court admissibility. Presumptively positive samples in Screening require a secondary confirmation.

If a sample is deemed presumptively positive during Screening, additional hair from the initial sample is taken for Extraction. In this phase, drugs are extracted from hair at a lower concentration than other methods (e.g., urine or oral fluid), which is why hair drug screening is among the most challenging techniques to conduct.

Confirmation of positive screens is done through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are pre-cleaned before confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
