An ideal test sample consists of a 1.5-inch segment of about 200 hair strands (comparable in size to a #2 pencil) nearest the scalp, yielding 100 mg, perfect for initial screening and verification. For EtG, additional tests, and/or those exceeding 10 panels, a 150 mg sample is advisable. Using a jeweler's scale is recommended to weigh the sample. If scalp hair is not available, the collection of an equal mass of body hair is permissible. In references to head hair, it specifically means scalp hair. Body hair denotes all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result encompasses four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the lab's system, ensuring the sample is sealed and transported correctly, generating a random LAN (Laboratory Accessioning Number), and completing any additional data input absent from an electronic chain of custody system.

Screening serves as a preliminary evaluation for drug presence. Though cost-efficient in excluding drug presence for most samples, a positive finding in Screening requires confirmation before legal admissibility. Samples with presumptive positivity in Screening necessitate secondary verification.

Should a sample test presumptively positive in Screening, further hair is extracted from the original specimen for the Extraction phase. Here, substances are removed from hair at notably inferior concentrations relative to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging technique to execute.

Verification of positive Screening outcomes is pursued using GC/MS, GC/MS/MS or LC/MS/MS techniques. Presumptive positive samples undergo washing prior to confirmation as required. The comprehensive lab process from Accessioning through to Confirmation is subject to review under the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
