What it is, How it works

When taken near the scalp, hair can reveal a history of alcohol and drugs up to approximately three months. It is straightforward to obtain, hard to tamper with, and convenient to mail.

A strand sample of 1.5 inches, comprising approximately 200 hairs (equivalent to the thickness of a #2 pencil), yields 100mg of hair, the optimal quantity for screening and confirmation. For EtG additions or more than 10-panel tests, a specimen amount of 150mg is suggested. We advise using a jeweler’s scale to weigh the sample. If scalp hair cannot be obtained, the same amount of body hair can be used. When mentioning head hair, we refer exclusively to scalp hair. Body hair includes other types (facial, axillary, etc.).

Process Overview

The main steps in lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a specimen within a laboratory’s system. This step verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry not covered by an electronic chain of custody process.

Screening is a preliminary quick test for drug substances. Although Screening is a cost-efficient method to identify drug usage in most samples, a positive result needs confirmation to be valid in court. Presumptively positive samples in Screening require secondary confirmation.

If a sample tests as presumptively positive in Screening, additional hair is retrieved from the original sample for Extraction. At this stage, drugs are extracted from hair at concentrations significantly lower than in other methods (e.g., urine or oral fluid), making hair drug testing the most challenging method to execute.

Confirmation of any positive Screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, presumptively positive samples are washed as necessary. The full laboratory process from Accessioning to Confirmation is overseen under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO/IEC 17025 standards.
