A 1.5-inch segment of around 200 hair strands (comparable to a #2 pencil's thickness) proximal to the scalp will yield 100mg, ideal for both screening and confirmation. For EtG, additional panels, and tests exceeding 10 panels, 150mg is advised. We suggest using a jeweler's scale for weighing the sample. If scalp hair cannot be obtained, an equivalent quantity of body hair can be used. When mentioning head hair, we mean scalp hair specifically. Body hair refers to all other types such as facial and axillary hair.

Process Overview

The four primary phases in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary handling of a sample into a lab's system. This step involves confirming that the sample was sealed and dispatched correctly, giving it a random LAN (Laboratory Accessioning Number), and completing any further data entry absent from an electronic chain of custody.

Screening entails an initial rapid inspection for drugs of abuse. Although Screening is an economical strategy to negate drug use on most samples, a positive result requires confirmation to be legally valid. Samples that show presumptive positives in Screening need further verification.

If a sample shows a presumptive positive during the Screening stage, extra hair is sourced from the initial sample and readied for Extraction. In this phase, drugs are taken from hair at considerably lesser concentrations than in other techniques (such as urine or oral fluid), making hair drug screening the most complex procedure to execute.

Confirmation for any positive Screening result is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as necessary. The entire lab sequence from Accessioning to Confirmation is evaluated under CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
