What it is, How it works

Hair testing is recognized as a potent method for detecting drug and alcohol consumption. Hair can furnish a lengthy history of alcohol and other substances by encasing biomarkers within the fibers of the developing hair strand. When harvested near the scalp, hair offers up to a roughly 3-month timeframe for detection of alcohol and other drugs. Hair is easy to collect, challenging to tamper with, and convenient to transport.

Collecting a 1.5-inch sample of roughly 200 hair strands (comparable to a #2 pencil's diameter) nearest to the scalp yields 100mg of hair, the prime amount for analysis and verification. For EtG, additional analyses, and/or tests beyond 10 panels, 150mg of the sample is suggested. We advise using a jeweler’s scale for weight measurement. If scalp hair isn't available, an equivalent quantity of body hair may be used. References to head hair pertain exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

There are four key stages in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary entry of a sample into a lab’s system. This process includes validating that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any supplemental data not supplied by an electronic chain of custody system.

Screening is a preliminary check for substances of abuse. While Screening is an affordable means to rule out drug use in most samples, a positive screen must be confirmed to be court-admissible. Any samples that test presumptively positive during Screening require additional confirmation.

If a sample is deemed presumptively positive at the Screening phase, additional hair is extracted from the initial sample and prepped for Extraction. Here, drugs are isolated from hair at much lower concentrations than in other methods (such as urine or oral fluid), making hair drug screening one of the more challenging methodologies to execute.

Confirmation of a positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples requiring confirmation undergo washing as needed. The entire laboratory process from Accessioning to Confirmation is conducted in compliance with both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Lengthy detection window: Hair drug tests detect drug usage up to 90 days, unlike urine tests which have a shorter detection span.
  • Challenging to deceive: Cheating a hair drug test is highly difficult, enhancing result accuracy.
  • Reveals historical usage data: Provides a drug use pattern over time, not just recent use.

Limitations:

  • Cannot spot recent usage: Detecting drugs in hair takes about 5-7 days after use.
  • Expense: Hair drug testing tends to be more costly than other methods.
  • Result variability: Variables such as hair color and individual hair growth differences can impact drug metabolite concentrations in the hair.

Note: The term "hair follicle tests" is a misnomer as the analysis targets the hair strand, not the follicle beneath the scalp
