Hair sample collection is straightforward, hard to tamper with, and easy to transport.

A 1.5-inch hair sample consisting of approximately 200 strands (roughly the thickness of a #2 pencil) closest to the scalp provides 100mg of hair, which is the optimal sample for initial testing and confirmation. For EtG, additional tests, and tests over 10 panels, a 150mg sample is advised. We suggest measuring the specimen with a jeweler’s scale. If scalp hair cannot be obtained, an equivalent amount of body hair may be used. Scalp hair refers specifically to head hair, while body hair includes all other hair types (such as facial and axillary hair).

Process Overview

The laboratory procedure for processing a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a lab's system. This step ensures that the sample was correctly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and completes any data entry missing from an electronic chain of custody system.

Screening provides a preliminary evaluation for the presence of abusive drugs. Although Screening is an economical method to exclude drug use for most samples, a positive screen must be confirmed for legal admissibility. Samples that show a presumptive positive in Screening require additional confirmation.

Should a sample show a presumptive positive during Screening, additional hair is extracted from the specimen for the Extraction stage. During Extraction, drugs are removed from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug testing is the most challenging method to execute.

Confirmation of any positive results from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that are presumptive positive undergo washing before confirmation as required. The entire laboratory process, from Accessioning to Confirmation, is conducted in compliance with both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation standards.
