For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advisable. Utilize a jeweler’s scale for weighing. In cases where scalp hair is unavailable, an equivalent volume of body hair may be used. References to head hair relate solely to scalp hair, while body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial entry of a sample into the lab’s system, verifying proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and entering any additional data not included in an electronic chain of custody system.

Screening is a preliminary, swift check for drug abuse. While it is a cost-effective means to exclude drug use in most samples, a positive screen requires confirmation for legal admissibility. Any samples testing presumptively positive in Screening necessitate further confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the initial specimen and readied for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), rendering hair drug screening the most complex methodology to conduct.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as necessary. The complete laboratory process, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
