If scalp hair isn't available, an equivalent amount of body hair can be used. "Head hair" refers exclusively to scalp hair, whereas "body hair" encompasses all other types (such as facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial incorporation of a sample into a laboratory's system. This process includes ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not recorded by an electronic chain of custody system.

Screening acts as a preliminary test to quickly determine the presence of substance abuse. While Screening is cost-efficient for excluding drug use in most cases, a positive result needs confirmation for legal admissibility. Samples that presumptively test positive in Screening require additional confirmation.

Should a sample indicate a presumptive positive in Screening, further hair is extracted from the initial sample for the Extraction process. Here, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging procedure to execute.

Confirmation of any positive Screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples that are presumptively positive undergo washing before Confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation standards.
