When taken close to the scalp, hair can reveal a nearly 3-month timeframe for detecting alcohol and other drugs. Hair is straightforward to obtain, relatively challenging to tamper with, and convenient to transport.

A 1.5-inch sample consisting of about 200 hairs (roughly the size of a #2 pencil) near the scalp will yield 100mg of hair, the optimal amount for both screening and confirmation processes. For EtG, additional tests, and/or panels exceeding 10, a specimen of 150mg is advisable. Using a jeweler’s scale to weigh the specimen is recommended. If scalp hair isn't available, an equivalent amount of body hair can be used. When discussing head hair, we mean scalp hair exclusively. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The four essential steps in laboratory drug test processing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is integrated into a lab’s system. It involves verifying the sample's proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data not given by an electronic chain of custody system.

Screening involves a preliminary swift check for substances of abuse. While Screening offers a cost-effective means to dismiss drug use in most samples, a positive result requires confirmation to be admissible in court. Any samples with a presumptive positive in Screening require secondary confirmation.

If a sample shows a presumptive positive result in Screening, more hair is extracted from the initial specimen and prepared for Extraction. At this stage, drugs are extracted from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, any presumptive positive samples are washed as necessary. The complete laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
