A sample measuring 1.5 inches, consisting of around 200 hair strands (comparable to the thickness of a #2 pencil) near the scalp yields 100mg of hair, which is considered optimal for screening and confirmation. For EtG, add-ons, and/or tests with over 10 panels, a 150mg sample is advisable. Utilizing a jeweler’s scale to weigh the specimen is recommended. If scalp hair is unavailable, an equivalent amount of body hair can be collected. Mentions of head hair pertain exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The process for analyzing a drug test result in a lab consists of four stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the stage where a sample is first processed into the laboratory’s records. This step verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes extra data entry not captured by an electronic chain of custody system.

Screening consists of an initial swift assessment for drug abuse. While this is a cost-efficient technique to eliminate drug usage in many samples, a positive outcome from a screen must be verified to be legally accepted. Any samples showing presumptive positive results during Screening require subsequent confirmation.

If a sample yields a presumptive positive result at the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. During this phase, drugs are extracted from hair at concentrations significantly less concentrated than other methods (e.g., urine or oral fluid), marking hair drug screening as the most challenging methodology to execute.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The complete laboratory process, from Accessioning to Confirmation, is reviewed with adherence to both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
