A 1.5-inch portion consisting of roughly 200 hair strands (about the dimension of a #2 pencil) nearest to the scalp supplies 100mg of hair, which is the optimal sample for both screening and verification. For EtG, supplementary, and/or assessments beyond 10 panels, a specimen quantity of 150mg is advised. We suggest measuring the specimen using a jeweler’s scale. If hair from the scalp isn’t available, a comparable quantity of body hair can be collected. When discussing head hair, we mean scalp hair exclusively. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The primary stages involved in lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the primary handling of a sample within a lab’s system. This involves confirming that the sample was securely sealed and shipped effectively, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any further data entry not facilitated by an electronic chain of custody process.

Screening encompasses an initial swift evaluation for drugs of misuse. While Screening is a budget-friendly method to exclude drug usage in most samples, a positive screen requires confirmation to be valid in legal proceedings. Any samples presumed positive during Screening necessitate a secondary confirmation.

If a specimen is presumptively positive at Screening, additional hair is drawn from the initial sample and readied for Extraction. During this phase, drugs are extracted from hair at considerably lower concentrations than in other methods (e.g., urine or oral fluid), explaining why hair drug screening is among the most challenging methodologies to conduct.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are washed before confirmation as necessary. The entire lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
