What it is, How it works

Hair analysis is regarded as an effective mechanism for detecting drug and alcohol consumption. Hair retains a long-term record of alcohol and other substances by capturing biomarkers within the fibers of the developing hair strand. When collected near the scalp, hair can reveal up to about a 3-month detection period for alcohol and other substances. Hair collection is straightforward, relatively hard to tamper with, and convenient to ship.

A 1.5-inch sample consisting of approximately 200 hair strands (roughly the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for screening and verification. For EtG, add-ons, and/or testing beyond 10 panels, 150mg of the sample is advised. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair is not present, a comparable amount of body hair can be used. When mentioning head hair, we are indicating scalp hair solely. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four primary stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample into a lab’s system. This stage includes ensuring the sample was sealed and delivered correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data input not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for substances of abuse. While Screening is an efficient method to exclude drug use on most samples, a positive screen must be verified to be permissible in legal proceedings. Any samples potentially positive in Screening need a secondary confirmation.

In cases where a sample is potentially positive during the Screening phase, additional hair is extracted from the primary specimen to prepare for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than other methodologies (e.g., urine or oral fluid), which makes hair drug screening the most challenging approach to perform.

Verification of any positive screening outcome is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are potentially positive undergo washing before confirmation as required. The full laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair specification and accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
