Collecting hair is straightforward, relatively tamper-resistant, and easily shippable.

A sample measuring 1.5 inches consisting of 200 hair strands (similar in thickness to a #2 pencil) from the scalp yields 100mg of hair, ideal for evaluation and confirmation. For evaluations involving EtG, additional factors, or exceeding 10 panels, 150mg is preferred. We advise using a jeweler’s scale for weighing the sample. If scalp hair is not available, the same quantity of body hair might be used. References to head hair pertain solely to scalp hair. Body hair includes all other hair such as facial, axillary, etc.

Process Overview

The procedure for laboratory processing of drug test results encompasses Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the lab's framework, ensuring proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening is an initial rapid evaluation for drug presence. Although cost-effective for excluding drug use in most cases, a positive screening must be substantiated for legal admissibility. Any presumptive positives identified in Screening require a follow-up confirmation.

Presumptive positives from Screening necessitate extracting more hair from the initial sample and preparing it for Extraction. At this stage, drug concentrations are extracted from hair at significantly lower levels compared to other methods (e.g., urine or oral fluid), rendering hair drug screening the most challenging method to conduct.

Confirmation of any positive screening results is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Samples with presumptive positives are cleansed as necessary before confirmation. The entire laboratory sequence from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
