An optimal test sample of 1.5 inches, consisting of around 200 hair strands (akin to a #2 pencil’s girth), will yield approximately 100mg, which is sufficient for both preliminary assessment and detailed confirmation. When analyzing for EtG, additional tests, and/or screenings exceeding 10 panels, a quantity of 150mg is favored. We advise using a jeweler’s scale to measure the specimen. In instances where scalp hair is not available, an equivalent quantity of body hair is acceptable. When mentioning head hair, we specifically refer to scalp hair. Body hair encompasses all other types, such as facial and axillary hair.

Process Overview

The laboratory procedure for evaluating a drug test outcome is divided into four significant phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the introductory processing of a sample into the lab’s system, ensuring the sample was correctly sealed and delivered, assigning an arbitrary LAN (Laboratory Accessioning Number), and finalizing any further data input not covered by an electronic chain of custody application.

Screening is an initial rapid assessment for illicit substances. Any samples yielding a tentative positive result during Screening mandate a subsequent confirmation.

For samples with a tentatively positive Screening result, additional hair is extracted from the main specimen for the Extraction process. In this stage, substances are retrieved from hair at substantially low concentrations compared to alternative methods like urine or saliva samples, underscoring the complexity of hair drug screening.

The Confirmation of any tentative positive during Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples identified as tentatively positive are cleansed before Confirmation as necessary. The comprehensive laboratory process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair specification and attains ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
