A 1.5-inch piece consisting of about 200 hair strands (nearly the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the preferred sample size for testing and confirmation. For EtG, additional analyses, and/or tests exceeding 10 panels, a specimen of 150mg is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not available, an equivalent quantity of body hair may be used. When mentioning head hair, we are specifically referring to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The four primary stages in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a laboratory’s system. It involves verifying that the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening provides a preliminary rapid review for drugs of abuse. Although Screening is a cost-efficient way to exclude drug usage in the majority of samples, a positive result in screening must be confirmed to be legally admissible. Any samples yielding a presumptive positive result in Screening need a subsequent confirmation.

If a sample shows a presumptive positive result in the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. During Extraction, drugs are isolated from hair at a much lower concentration compared to other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most complex methodology to execute.

Confirmation of any positive outcome from Screening is executed via GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples are cleaned before confirmation when necessary. The whole laboratory process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 certification standards.
